This notice explains how we may use and disclose your IIHI, your rights, and our obligations under federal and state law.

We may update this Notice; any changes will apply to records we already have as well as any we create in the future. The current Notice will be posted at our office and available on request.

B. How We May Use & Disclose Your Health Information

We may use and disclose your IIHI for a variety of purposes. Below are the primary categories and examples.

1. Treatment

We use and share information to provide, coordinate, and manage your medical care (e.g., test results, prescriptions, referrals).

2. Payment

We use IIHI to bill and collect payment for services. This may include communicating with your insurer or third-party payers.

3. Health Care Operations

We may use IIHI for quality assessment, staff training, audits, licensing, and business planning.

4. Appointment Reminders

We may contact you to remind you of upcoming appointments by phone, text, or email.

5. Treatment Options

We may inform you about alternative treatments or services that may be of interest.

6. Health-Related Benefits & Services

We may contact you about wellness programs, promotions, or other services offered by LuminousLuxe.

C. Use & Disclosure in Special Circumstances

We may be required or permitted to disclose IIHI for certain public or legal purposes, including but not limited to:

Public Health: disease reporting, product safety, exposure notifications.
Health Oversight: audits, investigations, licensure.
Legal Proceedings: subpoenas, court orders (with safeguards where required).
Law Enforcement: as required for investigations or public safety.
Research: limited circumstances with protections and approvals.
Deceased Individuals, Organ Donation, Military, Inmates, Workers’ Comp: as permitted or required by law.
Serious Threats: disclosures necessary to prevent a serious threat to health or safety.

D. Your Rights Regarding Your Health Information

Confidential Communications: You may request that we contact you in a specific way or at a specific location. Submit requests in writing to the contact listed above.

Request Restrictions: You may request limits on certain uses and disclosures. We are not required to agree, but will do so when required by law.

Inspect & Copy: You may request access to inspect and obtain copies of your IIHI (fees may apply). Some limits may apply by law.

Amendments: If you believe your records are incorrect, you may request an amendment. We may deny the request under certain circumstances.

Accounting of Disclosures: You may request a list of non-routine disclosures for up to the last six years. One accounting is free per 12-month period.

Right to a Paper Copy: You are entitled to a paper copy of this Notice upon request.

Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with LuminousLuxe at the contact listed above or with the U.S. Department of Health & Human Services. You will not be penalized for filing a complaint.

E. Authorizations, Revocation & Contact

Uses and disclosures not described in this Notice generally require your written authorization. You may revoke an authorization in writing at any time, except to the extent that we have already acted based on your authorization. We are required to maintain records of care.
